Comprehensive Rehabilitation Program...

 

Lakeside Rehabilitation has all of the "conveniences of home," including a washer/dryer, bathtub, shower and full kitchen with dining so that an individual may regain the highest possible recovery and independence. Clients, by special request, have also been taken grocery shopping on occasion to ensure independence prior to discharge.

 

Lakeside has found that the inter-disciplinary team is the best approach to provide the highest quality of care for rehabilitation patients. From the specially trained nurses to our therapists who follow each client carefully, Lakeside 's team approach has proven efficient and effective in supporting our mission of "Quality Care Close to Home".

 


There is an array of issues to consider when seeking a rehabilitation program. The interdisciplinary team is a primary factor in the success of your rehabilitation.

 

Clients are referred to us while hospitalized. Afterwards, they are admitted to the program under the watchful eye of a primary care physician and the team of nurses and therapists who administer the care.

 

Our inter-disciplinary team then sets up a complete daily plan as well as a tentative discharge date, for each individual. Therapy is followed closely by all team members and each case is discussed to share progress once a week. At that time, changes in the treatment plan are made and all staff are notified.

 

Discharge planning begins at the time of admission. A representative may accompany the client or a family member to the client's home to determine what barriers will be addressed and eliminated before a successful return home can be achieved. A barrier might be an upstairs bedroom, so ability to navigate stairs safely would be an important rehabilitation goal. When all goals are met by the client, he or she can be released home or to a lower level care facility as needed.
